What Is a Brachial Plexus Injury?

The brachial plexus is a network of nerves (C5–T1) running from the spinal cord through the neck and shoulder. It controls every movement and sensation in the shoulder, arm, and hand.

When these nerves are stretched, compressed, or torn — most commonly in road accidents — the consequences range from temporary weakness to complete arm paralysis.

Common Causes

  • Road & motorcycle accidents (most common cause)
  • High-impact sports injuries
  • Birth trauma (neonatal BPI)
  • Falls and direct trauma
  • Tumors or nerve inflammation

Warning Symptoms — Do Not Ignore

If you or someone you know has experienced trauma and shows any of these signs, seek specialist evaluation immediately.

Arm weakness or no movement
Burning / electric-shock pain
Loss of grip strength
Shoulder stiffness
Numbness or loss of sensation
Partial or complete paralysis
Advanced Diagnostics at Surgenesis:
  • MRI Brachial Plexus (high-resolution)
  • EMG (Electromyography)
  • NCV (Nerve Conduction Velocity) Test
  • Detailed Neurological Clinical Evaluation

Types of Brachial Plexus Injuries

Not all brachial plexus injuries are the same. Correct classification is critical — it determines whether surgery is needed and which procedure will give you the best outcome.

Severe

Root Avulsion (C5–T1)

The nerve root is torn directly from the spinal cord — the most severe form. Direct repair is not possible. Nerve transfer surgery is required to re-route healthy nerves to restore function.

Mild

Neuropraxia

A temporary nerve stretch with no structural damage. The nerve is intact and many patients recover spontaneously without surgery, though close monitoring is essential.

Moderate

Lateral Cord Injury

Affects the musculocutaneous nerve, leading to weak elbow flexion and reduced forearm function. Surgical intervention is often beneficial for functional recovery.

Moderate–Severe

Rupture / Neuroma

The nerve is torn but not from the spinal cord. Scar tissue (neuroma) forms and blocks nerve signals. Nerve grafting or direct repair is typically required.

Treatment Options at Surgenesis

Every treatment plan is personalized based on the injury type, time elapsed since injury, and patient goals. Dr. Amit Mittal uses the latest microsurgical techniques available in India.

01

Nerve Transfer Surgery

The most effective treatment for root avulsion injuries. A healthy functioning nerve is redirected to restore movement where the original nerve is beyond repair.

  • SAN → SSN Transfer (shoulder function)
  • Oberlin Transfer (elbow flexion)
  • Intercostal nerve transfers
02

Nerve Grafting

When nerves are torn but not avulsed, a graft bridges the gap between the two ends. The nerve regenerates through the graft over time, restoring signal transmission.

  • Sural nerve graft
  • Vascularized nerve graft
  • Combined graft procedures
03

FFMT – Free Functioning Muscle Transfer

For long-standing paralysis where muscles have wasted, a healthy muscle from elsewhere in the body is transplanted with its nerve and blood supply to restore movement.

  • Gracilis muscle transfer
  • For late-presenting cases
  • Restores elbow/finger function
04

Tendon Transfer

Functioning tendons are rerouted to perform the job of paralyzed ones — restoring grip, pinch, and hand movements without waiting for nerve regeneration.

  • Restores hand function
  • Faster functional recovery
  • Combined with nerve procedures

A detailed surgical and rehabilitation plan is created specifically for your injury type, age, and functional goals.

4

Microsurgery

Procedure performed under high-powered magnification by Dr. Amit Mittal using advanced microsurgical instruments.

5

Rehabilitation Program

Structured physiotherapy, muscle re-education, and hand training begin as soon as healing allows — critical for maximizing outcomes.

6

Long-Term Recovery Tracking

Regular follow-up assessments with Dr. Mittal to monitor nerve regeneration and adjust the rehabilitation plan.

Recovery Timeline & Why Timing Matters

Nerve regeneration is a slow process. Understanding the timeline sets realistic expectations and underscores why early treatment is non-negotiable.

Recovery Milestones

6–8
wks

Initial Healing

Surgical wounds close and initial nerve repair consolidates. Gentle mobilization and swelling management begin.

3–6
mo

Early Nerve Signals

First signs of nerve regeneration may appear. Physiotherapy becomes more active. Muscle re-education starts.

6–24
mo

Functional Recovery

Progressive return of movement, strength, and grip. Recovery continues improving for up to two years post-surgery.

⚠️ Consequences of Delayed Treatment

Surgery after 6 months produces significantly worse outcomes. Beyond 12–18 months, some damage becomes irreversible.

  • Permanent muscle wasting (atrophy)
  • Irreversible joint stiffness and contracture
  • Severely reduced surgical options
  • Chronic nerve pain that is difficult to treat
  • Permanent partial or complete paralysis

Rehabilitation Includes:

  • Specialist physiotherapy program
  • Progressive muscle strengthening
  • Occupational & hand therapy
  • Long-term neurological monitoring

Frequently Asked Questions

Answers to the questions we hear most often from patients and families.

What exactly is a brachial plexus injury? +
The brachial plexus is the network of nerves (C5–T1) that controls movement and sensation in the shoulder, arm, and hand. Injury occurs when these nerves are stretched, compressed, or torn — most commonly in high-speed road accidents. Consequences range from temporary weakness to complete paralysis of the arm, depending on severity.
When is brachial plexus surgery actually needed? +
Surgery is recommended when there is no spontaneous recovery within 3–6 months, when imaging confirms severe nerve damage (rupture or avulsion), or when there is significant loss of arm or hand function. Mild neuropraxia injuries may recover without surgery, but require monitoring. Early specialist evaluation is critical — don't wait to see if it "gets better on its own."
Can brachial plexus injury be fully cured? +
Mild injuries (neuropraxia) often recover completely. Severe injuries — particularly root avulsions — cannot be fully reversed, but advanced procedures like nerve transfer and FFMT can restore significant functional movement and independence. The goal is maximum practical recovery, not theoretical perfection. Most patients achieve meaningful improvement in daily function.
What is nerve transfer surgery and how does it work? +
Nerve transfer (neurotization) takes a healthy, functioning nerve from a nearby region and reroutes it to power a paralysed muscle. For example, the Oberlin transfer uses a branch of the ulnar nerve to restore elbow flexion, while SAN to SSN transfer restores shoulder movement. The brain gradually re-learns to use the repurposed nerve pathway through rehabilitation.
How long does recovery take after surgery? +
Initial wound healing takes 6–8 weeks. Nerves regenerate slowly — roughly 1mm per day — so meaningful functional recovery typically takes 6 months to 2 years. Consistent physiotherapy during this period is not optional; it is what converts nerve regeneration into actual muscle strength and movement.
What does brachial plexus surgery cost in Jaipur? +
Cost depends on injury severity, the procedure required (nerve transfer, grafting, or FFMT), hospital stay duration, and rehabilitation needs. Surgenesis Hospital offers significantly lower costs than equivalent centres in Western countries, while maintaining the same standard of surgical expertise. Contact us for a personalized cost estimate after initial consultation.
Do you treat international patients? +
Yes. Dr. Amit Mittal regularly treats patients from outside India. We offer initial video consultations, help coordinate travel logistics, arrange nearby accommodation, and provide teleconsultation follow-up after patients return home. Contact us to start the process remotely.
